Applied pays 100% of the employee premium for a PPO-based plan medical health insurance. Spouse/Dependents can be added at cost and are paid via pre-tax payroll deductions.

Voluntary Benefits
All new employees are eligible for Dental, Vision, Life/AD&D, Accident, Short-Term Disability, Long-Term Disability, Critical Illness, and Hospital Indemnity at first of the month following 60 days from hire date.
Spouse and dependents can be added to the Dental and Vision, Life/AD&D, Accident, Critical Illness, and Hospital Indemnity.
Employees will be responsible for all Voluntary Benefits Premiums.

401(k) Plan
Applied offers a 401K with ROTH option. Applied is a safe-harbor plan, matching 100% of employee's contribution up to 4% of gross taxable wages. (IRS limits apply.) Eligibility period is after a minimum of 90 days employment.
